For the base layer we typically use crusher run or 21-A compactable gravel. For the surface we recommend #57 stone or a similar clean angular stone that compacts well and doesn't track. We'll recommend the right type based on your soil conditions and traffic level during the site visit.
Almost always a drainage problem. Either the driveway doesn't have a crown or pitch to move water off the surface, water is running down the driveway from uphill, or there's no culvert at a low point to redirect water. We'll diagnose the exact cause on the site visit and show you what's needed to fix it permanently.
